The study employed qualitative research approach informed by case study design. It deployed purposive sampling techniques. A total number of 15 participants were involved in the study. This research employed two data collection techniques namely unstructured interviews and document analysis. Collected data were analysed qualitatively firstly by transcribed the audio recorded information and thereafter the transcribed data was subjected to inductive data analysis. From there, patterns themes and categories were generated which resulted into interpretation stage as per research objectives. In general the study found out that most of health projects are designed in outreach system in nature. Therefore, when project duration comes to an end the host institutions failed to meet outreach cost and capacity to buy health equipments. Furthermore, there was less participation of politicians particularly in upper level structure of management on planning stage of health projects rather than were being involved the implementation stage of public health projects.
